Our Services
Home care nurses have an opportunity to profoundly and
positively affect the lives of their patients by providing
care and services that allow the patient to stay at home,
optimizing quality of life and staying within the comfort
of home and family. Our nursing staff works collaboratively
with an interdisciplinary team that can include pharmacists,
physical, occupational, and/or speech therapists, medical
social work, chaplain services, and others to provide care
and services to patients in the home or alternate site setting.
Working independently, they are the eyes and ears for the
physician, managing the plan of care and providing updates
and clinical assessments with suggestions for modifications
as needed.

With home care services focusing on general home health,
home infusion, and hospice services, DHCC has a variety
of clinical focus areas to match your particular interest.
Experienced home care nurses are strongly desired, but DHCC
is also committed to furthering the advancement of home
care nursing with a new generation of skilled and highly
