Occ Summary

Responsible for coordinating continuum of care activities for assignedpatients and ensur ing optimum utilization of resources, servicedelivery and compliance wit h external review agencies' requirements.


Work Performed

Coordinate and facilitate timely implementation of discharge plans forassigned patients; arrange follow-up care as appropriate.Maintaineffe ctive communication with health care team members related todischarge planning and utilizationmanagement.Documentfindings, discharge arrangemen ts and actions taken according todepartmental guidelines; prepare report s and maintain records asrequired.Act as an educational resource and pro videconsultation to hospital medicalpersonnel regarding discharge plann ingprocess and applicable federal, state and local regulations; identify benefits, implications and limitations of home care as appropriate.Provi de concurrent review and follow up with appropriatepersonnel regarding denials.Collect denial data fortrendingWork with appropriate staff to dev elop processes toprevent denials and improve reimbursement.Collect analy ze andreport data for resource utilization, compliance, QI and other are as asassigned.Provide feedback thatwill enhance negotiations withpayers .Develop and maintain positive relationships withcustomers internal and external to Duke Health System.Performother relatedduties incidental to the work described herein.

Minimum Qualifications


Education

BSN or MSW required


Experience

3 years of relevant experience


Degrees, Licensures, Certifications

Requires ACM or CCM certification within 3years of hire. BSN must have current or compact RN licensure in the state of North Carolina. MSW must have a current North Carolina license as a CMSW within 90 days of hire or a current license as an LCSW.
