Duke Raleigh Hospital Palliative Care Nurse Practitioner Position

Duke Palliative Care provides compassionate care, innovative teaching and pioneering research to relieve the suffering for patients with serious illness and their loved ones.

Duke Palliative Care provides a variety of clinical services for patients with serious or life-limiting illness at Duke Raleigh Hospital (DRaH). DRaH is a 186-bed facility that is recognized as a community leader in compassionate care and with distinct centers of clinical excellence in oncology, orthopedics, neurosciences, and cardiovascular services. Palliative care services at this site are being expanded to include outpatient as well as the continuation of inpatient palliative care. This dynamic role for a Nurse Practitioner will provide the opportunity for developing a strong outpatient palliative care presence as well as play a vital role on the inpatient palliative care team.

Position Summary:
The Palliative Care Nurse Practitioner provides direct patient care services in the ambulatory and inpatient settings. The Nurse Practitioner performs physical exams, collects and documents data, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, under the supervision of the supervisory physician. The Palliative Care Nurse Practitioner contributes to excellence in patient care, research, teaching, and provides leadership in the organization.

Job Description
Assumes responsibility and accountability for the care of patients with serious or life-limiting illness in need of palliative care support in the inpatient setting at Duke Raleigh Hospital and in the outpatient setting through the Duke Raleigh Cancer Center. Will provide services under a collaborative practice agreement with the physicians of the palliative care practice.

Essential Job Functions/Responsibilities
 Performs accurate history and physical examinations for patients in the inpatient and outpatient setting.
 Initiates treatment of physical symptoms within the nurse practitioner’s scope of practice.
 Completes timely and accurate documentation of all activities in the EMR system.
 Provides assistance to patients and families in goal setting and care plan development with end-of-life discussions.
 Plans care with inpatient and outpatient social work and case managers, utilizing community resources (including home health and home hospice agencies) and acts as a liaison by relaying information to other health care professionals and agencies to provide for continuity of care.
 Participates in education of other providers and promotes culture transformation to reinforce compassion and support for patients within the health system.
 Maintains consistent and on-going communications with the consult physician regarding patients and recommendations.
 Promotes patient relations through confidentiality, privacy, and dignity. Provides services in a quick and courteous manner with care and respect for the patient’s individual needs. Maintains a professional appearance and a clean, safe working environment.
 Communication
 Consults with the collaborating physicians within the practice according to the collaborative practice agreement.
 Communicates patient/family goal needs to staff and attending physicians.
 Communicates with bedside nursing staff regarding goals and treatment plans.

Schedule Structure
 Inpatient Consultation Team
o Provides inpatient palliative care consultations and rounding in collaboration with the consult physician.
 Outpatient Clinic
o Performs new patient consultations and follow-up/continuity visits

Position Requirements
 Completion of accredited Nurse Practitioner program
 State of North Carolina licensure
 Minimum of three years’ experience providing clinical care to patients with complex medical problems and/or providing end-of-life care.
 Demonstrated competency in pain, stress, and other symptom management.
 Current ACHPN certification or ability to obtain after one year
 DEA and NPI
 Current BLS certification
 Strongly prefer candidates with at least one year experience as a palliative care NP.

Excellent benefit package. Primary hours will be weekdays from 8:00am-5:00pm. No call. Salary commensurate with experience and qualifications.
